NOTE
- It is recommended that you begin preparation as early as possible.
Medical examination and safety training for radiation workers are essential.
NIMS employees must contact the NIMS Safety Control Office for document approval. - Registration for Radiation Work is valid for each fiscal year, from April to March next year, and automatically ends on March 31st. Registration must be renewed every fiscal year.
- The Project Leader is responsible for document submission for all team members.
- Those who submit the document after the BL15XU Office's deadline cannot attend the experiments.
- Correction to entries should be made by strike-through and signature, and leave the original entry still readable. The use of correction tape/pen is not acceptable.
- From the term 2014A, every user including those affiliated with an organization outside Japan is required to submit the original document. The electronic files (pdf) are not acceptable.
How to prepare the Radiation Worker Registration Form
- Click the link "To prepare the Radiation Worker Registration Form (RIKEN)" mentioned below.
- Click "SPring-8/SACLA User".
- Fill in the form.
* Be sure that all entries are consistent with what are registered in the SPring-8
user registration information.
* Concerning the period work schedule, enter the date you visit SPring-8 and the
last date (March 31) of this fiscal year.
- Click "OK" on the bottom of the page.
- Check the preview and click "Next" on the bottom of the page.
- Click "Create PDF".
- After printing out the document double-sided, fill in the necessary information and put the seal/signature on it.

Those affiliated with RIKEN or JASRI
For those who have a position in RIKEN or JASRI (Japan Synchrotron Radiation Research Institute), radiation worker registration procedures might differ from those for general users. Please ask your administrative secretary at RIKEN/JASRI for the procedures you should carry forward.
Those whose affiliation will change during a fiscal year
Please submit the Radiation Worker Registration Cancellation Form [Form 5-2], and submit a new Form 5-1.
